VA CLC's Ongoing National Center for Enhancing Resources and Training (CONCERT)

CONCERT helps build key infrastructure to support Geriatrics and Extended Care's vision for system-wide improvement in staff engagement, Veteran experience, and person-centered care. It comprises a large, multi-disciplinary team based at CHOIR and the Tuscaloosa VA Medical Center. In fiscal years 2016 and 2017, CONCERT prepared for and conducted the national rollout of the Bright Spots Program to Veterans Health Administration (VHA) Community Living Centers (CLCs—nursing homes). The Bright Spots Program engages and supports CLC staff in creating high-functioning, relationship-based teams to improve staff egagement and focus on person-centered care, thereby improving the Veteran experience. The program builds on existing literature emphasizing the importance of collaboration and team-building, structured observation, and strengths-based learning. It capitalizes on Drs. Hartmann and Snow's studies funded by VA's Health Services Research & Development service, including work done as part of the Long Term Care CREATE, and a project funded by VA's Office of Patient-Centered Care and Cultural Transformation. CONCERT also serves as the umbrella center for VISN-level support for CLCs and ongoing VISN-level webinars. Since fiscal year 2018, CONCERT also facilitates in-person and virtual learning intensives for CLCs that are modeled on the Institute for Healthcare Improvement Breakthrough Collaborative Series. CONCERT represents a strong operations-research partnership and a step forward on the path to achieving VHA's priorities for an engaged workforce in a learning health care organization with top-quality service and highly reliable, person-centered care.

Under VA’s Facility-Based Care Modernization Plan, CONCERT will move to VA Central Office. Within VA’s Office of Geriatrics and Extended Care, there will be one full-time CONCERT lead position along with four full-time CLC-focused and four full-time SVH (State Veterans Home)-focused CONCERT staff.
